N-TERMINAL PRO-BRAIN NATRIURETIC PEPTIDE LEVELS IN PATIENTS WITH EARLY AND LONG-TERM RHEUMATOID ARTHRITIS

TOPIC: Cardiovascular Disease TYPE: Original Investigations PURPOSE: N-terminal pro-brain natriuretic peptide (NT-proBNP) is a recognized predictor of heart failure and cardiovascular death. Rheumatoid arthritis (RA) patients (pts) were shown to have higher than in general population NT-proBNP concentrations, it unknown whether the level depends on RA duration, activity or treatment.Objective: To compare levels early untreated long-term pts with high disease activity. METHODS: A total 115 (77,4% females, 22,6% males, 56 [46;61] years old) was enrolled study: 67 (disease duration ≤12 months) 48 (>12 months). All glucocorticoids disease-modifying antirheumatic drugs (DMARDS) - naïve prior inclusion. Long-term treated (52,0%), methotrexate (72,9%), leflunomide (20,8%) sulfasalazine (12,5%). Serum (pg/mL) measured using electrochemiluminescence test Elecsys proBNP II (Roche Diagnostics, Switzerland). > 125,0 pg/mL considered as elevated. RESULTS: Median concentration whole group 91,4 [49,4;153,4] pg/ml. positively correlated age (r=0,4, p<0,0001), CRP (r=0,23, p=0,01). Pts comparable terms age, sex body mass index. had DAS28 scores (6,2 [5,4;6,7] vs 5,5 [5,1;6,4], p=0,01) (38,7 [11,0; 80,8] mg/L 22,6 [7,6;44,6] mg/L, p=0,03). Elevated concentrations found 46,3% 20,8% stage (р<0,01), 118,9 [64,2;198,0] 82,2 [42,1;113,6] pg/mL, respectively (р<0,01). Correlations between CRP, remained significant for both stages. CONCLUSIONS: Untreated rather >12 months resistance intolerance DMARDS. Although inflammation advanced led increased cases.
